I believe videos with the most activity are "suggested". This may be "negative" activity, youtube doesn't care. If a video has lots of recent views, comments, thumbs up OR down votes, it will be suggested. The point is to get people to stay on youtube watching ads.

If you did have an active youtube account, the video suggestions would tend to be more relevant to you. I rarely pay attention to these suggestions because I go straight to my subscriptions page. And I use two different ad blockers at the same time, so I never see the ads.
